Entry One. SELF-ASSESSMENT

 

Entry One: Self-Assessment.

Macro skill: Vocabulary.

 Age of the students: 4 years old.

 Topic: Daily Activities.

Performance Indicators: The child can name and describe their daily activities.

Assessment Resource: A worksheet with pictures depicting various daily activities.

 Method:   The teacher will distribute  each child a worksheet and ask them to circle the activities

 they do everyday. Then, have them describe one of these activities in detail.  The teacher will observe

 and note the range of vocabulary used by each kid.

Time: 20 minutes.

Materials: Live worksheets with daily activities, markers and notebook.

Entry Two. PERFORMANCE TASK

ENTRY 2: Performance Task.

Macro skill: Reading.

Age of the students: 4 years old.

Topic: Story Comprehension.

Performance Indicators: The child can retell the main events of a short story in

sequence.

Assessment Resource: A simple, illustrated short story.

Method: Teacher will read a story to the class. After reading a short story to the class, the teacher will ask individual students to retell the story in their own words. Teacher will note their ability to recall key details and sequence events.

Time: 20 minutes.

Materials:  Book with a short story. 




There was once a hare who was friends with a tortoise. One day, he challenged the tortoise to a race. Seeing how slow the tortoise was going, the hare thought he’d win this easily. So, he took a nap while the tortoise kept on going. When the hare woke, he saw that the tortoise was already at the finish line. Much to his chagrin, the tortoise won the race while he was busy sleeping.

Entry Three. DEMONSTRATION

 ENTRY 3: Demonstration.

Macro skill: Phonics.

 Age of the students: 4 years old.

Topic: Letter Sounds.

 Performance Indicators: The child can correctly demonstrate the sound associated with

each letter of the alphabet.

Assessment Resource: Alphabet flashcards.

Method: Show each student flashcards of different letters and ask them to produce the

corresponding sound. Record which sounds each child correctly identifies.

Time: 20 minutes.

Materials:  Flashcards with letter and a Youtube video.










Entry Four. OBSERVATION


ENTRY 4: Observation.

Macro skill: Listening.

Age of the students: 4 years old.

Topic: Animal Sounds.

Time: 20 minutes.

Performance Indicators: The child correctly identifies animals by their sounds.

Assessment Resource: Audio clips of different animal sounds.

 Method:  The teacher will play a song  to heard with the kid then the teacher will play various animal sounds and observe which children can correctly identify  the animals. Teacher will take  notes on the accuracy of their responses.

Material: A Youtube video.









Entry Five. QUESTIONS AND ANSWERS

 ENTRY 5: Questions and Answers.

 Macro skill: Speaking.

Age of the students: 4 years old.

Topic: Colors.

Performance Indicators: The child is able to correctly name colors when presented with

various objects.

Assessment Resource: Set of colored flashcards representing different colors.

Method: The teacher will distribute the worksheet  and markers to each kid. The teacher will ask each

 student to name the color of flashcards  and paint it with the correct color. The teacher will record the

 colors they correctly identify.

Materials:  Live worksheet that have different colors, markers, etc.  The techer will print the sheets and

 give them to the childrens,
